Dr. Somasundaram Sathappan is a Consultant Plastic and Reconstructive Surgeon and heads the Soma Plastic Surgery (Clinical Aesthetics) Clinic in Subang Jaya, Malaysia. Dr. Soma received his medical degree from the University of Malaya in Kuala Lumpur in 1989 and was posted to Sarawak in Malaysia Borneo for a year and half before returning to Kuala Lumpur and joining the National University of Malaysia as a trainee lecturer in the Department of Surgery. Dr. Soma completed his Fellowship of the Royal College of Surgeons in Edinburg, Scotland in 1994 and returned to the National University of Malaysia for a Masters Degree in Surgery in 1996, followed by additional plastic surgery training at the Welsh Centre for Burns and Plastic Surgery in Swansea, Wales which he completed in 2001.


A special interest in treating burns, scars and keloids led Dr. Soma to help establish the Advanced Burns Centre at the National University of Malaysia. He is a member of the Asian Scar Advisory Board, with the aim of advancing the science of scar and keloid treatment. Dr. Soma is also a member of International Society of Aesthetic Plastic Surgery (ISAPS), American Society of Plastic Surgery (ASPS) and Malaysian Association of Plastic, Aesthetic Craniomaxillofacial Surgery (MAPACS). Drawing earlier experience as a lecturer, Dr. Soma guides third and final year medical students at the National University of Malaysia and the post-graduates in the Masters in Surgery program in clinical bedside courses. Dr. Soma also teaches courses for student nurses as the University of Malaysia's teaching hospital and lends his expertise internationally as a tutor in the Royal College of Surgeons of Edinburgh's Basic Surgical Skills course.


Dr. Soma has lectured at many local and international meetings, especially on scars and scar treatment and stem cell facial rejuvenation using platelet rich plasma. Dr. Soma keeps a foot in the research community and actively participates in clinical study and experiments. What is Breast Reduction?

Breast Reduction, also known as Reduction Mammaplasty is a surgical procedure which removes the excess fat and skin from the breasts, which are reshaped and the nipples repositioned to form newer small breasts.

Who is the ideal candidate?

The ideal candidate will be physically healthy woman who wants to enhance the body contour for personal reasons, feels their breast is too big & those who wants to correct asymmetry of their breasts. This procedure also ideal for those who having their backache and infection of their inframammary fold.

How the procedure done?

Breast reduction surgery is usually done under general anesthesia. The procedure includes surgery through incisions or liposuction to remove the excess fat in your breasts. The surgeon usually makes an incision around the areola and down each breast, removes excess breast tissue, fat and skin to reduce the size of each breast and then reshapes the breast and repositions the nipple and areola. The nipple and areola usually remain attached to the breast but might need to be removed and then reattached at a higher position as a skin graft if your breasts are very large. After surgery, your breasts will be covered with a gauze dressing or bandages. A tube will be placed under each arm to drain any excess blood or fluid. This procedure takes 3 to 4 hours. The patient needs to be hospitalized 2 to 3 nights.

What can you expect after the operation?

Soreness and swelling are likely to be encountered for 1 to 2 weeks after surgery. Bruising is also possible. Any discomfort can be controlled by medication prescribed by your doctor. This procedure might leave scars on the breast or in the armpit & this varies between different individuals. You will be given a cream to help the scar heal well. You will be wearing a compression garment and a surgical bra after the surgery. Stitches can be taken out in a week to 10 days.

Breast Augmentation also known as Augmentation Mammoplasty is a surgical procedure using implants to enlarge the breasts by one or more cup sizes. A breast implant is made up of high grade silicone gel. This procedure can also restore the breast volume lost after weight reduction or pregnancy, achieve a more rounded breast shape or improve the natural breast size asymmetry. This procedure can be combined with Abdominoplasty, Liposuction & Facelift.

How the procedure done?

Breast augmentation surgery is done under general anesthesia as a day procedure. This procedure is done by placing an implant usually inserted through incisions made either under the breast (inframammary fold), in the areola or in the armpit (axillary). Once the implant is placed, the surgeon will close the incision with sutures.

What is Liposuction?

Liposuction, also known as liposculpture or lipoplasty is a procedure that can help sculpt the body by removing unwanted fat from almost all areas of the body. The common areas are the abdomen, thighs, hips, buttocks, calves, breasts, arms and the face. This can be combined with Bilateral Breast Augmentation, Abdominoplasty (Tummy Tuck), Facelift and Bilateral Breast Reduction. Liposuction procedure is not a treatment for obesity.

What are the conditions/prerequisites?

A good quality skin that is undamaged and able to retract and re-drape well after the fat has been removed. If the skin is of poor quality, it will hang down in folds after the liposuction.

How the procedure done?

The procedure involves pre-operative markings that will encompass areas of concern to the patient (i.e. areas of fat deposition that needs to be treated). The procedure is done under general anaesthesia. A solution of local anaesthetic with adrenalin is injected into the areas to be treated. This is to distend the fat tissues so that the surgery is easier and also to minimize bleeding and to ensure post-operative pain relief. A small hollow steel tubes measuring 3 to 4 mm in diameter are inserted at specific sites that are well hidden within skin creases and connected to a machine that will remove the fat. The amount of fat that can be removed at a single sitting safely varies from 3 to 6 litres depending on the individual patient. This procedure takes about 2 to 3 hours.

What can you expect after the operation?

After the operation, the treated areas are bandaged and a tight pressure garment is put on to compress the skin onto the deeper tissues. This helps prevent fluid collections and to ensure an even contour of the skin. This has to be applied for at least 6 weeks. You can go back to normal activities after 1 week and exercises can be commenced after 6 weeks. You might need to stay only 1 night at the hospital. Besides, complications are minimal and consist mainly of bruising that will resolve spontaneously over a period of 2 to 3 weeks. Infection is rare.

What is the significance of the procedure?

Fat cells are thought not to be regenerated in adult life. Therefore, their removal by liposuction will give a permanent change in contour & be independent of any changes in your body weight. Liposuction is a very safe procedure if it is done by certified & experienced surgeon in a major hospital with a well-trained anesthetist.
